Are you working on private land in Clark County that has not yet paid into the DCP/MSHCP?

Is a federal agency carrying, approving, or funding the project?

Is an EA or EIS going to be done for the entirety of this specific phase of the project?

Do you discharge water to WOTUS?

Is the project activity confined to routine maintenance that disturbs less than 5 acres that is performed to maintain the original line and grade, hydraulic capacity, or original purpose of a facility or structure?

Is there potential to discharge pollutants to WOTUS related to stormwater during or after construction (typically YES)?

Will there be dredged or fill material placed in a WOTUS?

Is there potential to discharge dredged or fill material in a WOTUS?

For projects discharging to WOTUS, will the discharge be clean water only AND one of the below discharge categories?

• Public Water System Emergency discharges
• Water supply systems discharges
• Well testing & maintenance/aquifer testing/water quality testing
• Subsurface water discharges
• Utility vault water discharges
